Delete your Clinic in a Box account and data

This is the account- and data-deletion request page for the Clinic in a Box app (package com.globalaudiologyconsulting.clinicinabox), published by Global Audiology Consulting LLC.

How to request deletion — steps:

  1. From the email address on file for your account, email drledee@globalaudiologyconsulting.com with the subject line "Delete my Clinic in a Box account and data".
  2. Include your name, your clinic / practice name, and whether you want (a) only your staff user account deleted, or (b) the entire practice account and its data deleted. Practice-wide deletion can only be requested by a practice administrator.
  3. We confirm your identity by replying to the address on file, then process the request within 30 days and email you when it is complete.

What is deleted: your user profile and login, the practice profile and branding, patient records and clinical notes, messages, appointments, claims and invoices, uploaded documents and consent PDFs, credentialing records, and push-notification tokens.

What is kept, and for how long: billing and payment records held by our payment processor (Stripe), and any records we are required to keep by law, by our Business Associate Agreement with the clinic, or under a legal hold. These are retained only for as long as that requirement lasts, then deleted or de-identified — see section 5 of our Privacy Policy. Copies in routine backups age out on our normal backup cycle.

If you are a patient: your health records belong to your clinic. Contact your healthcare provider directly to exercise rights over those records; the clinic instructs us as its HIPAA Business Associate.
